Long Term Care

The Long Term Care module serves the clinical, regulatory and reporting needs of long-term acute care (auxilliary hospital) and skilled nursing facilities. Part of the Epic integrated clinical information system, Long Term Care provides access to the patient's complete medical record using views and tools emphasizing long term care plans, minimum data set compliance, medication adjudication, outcomes tracking and billing for long-stay residence fees. A single patient story bridging the continuum of care leads to better-coordinated care, particularly when clients require transfer to or from acute care facilities.
