Record of Care

Where the 'health record' accounts for all aspects of health service provision, including matters of scheduling, administration, finance, diagnostic and treatment interventions, etc.; use of the phrase "record of care" connotes a record with official standing as the definitive account of care provided by identifiable health care provider(s), teams, groups or organizations. Questions about care planning, provision, process and outcomes should be answerable by reference to the record of care.
